My yoga mat has seen me at some of my lowest physical points — a back that would seize without warning, an ankle rebuilding itself bone by bone after a fracture, and a mind that some days would not stop long enough for me to feel anything at all. It has also seen me get better. Not in a straight line, and not overnight, but in the slow, repeatable way that actual healing tends to look.
I talk to patients about hormones and mental health all day. What I do not always say out loud is that one of my own most effective tools for both has been a yoga mat and twenty minutes.
What Chronic Back Pain Actually Taught Me
For years, my back was the thing that decided what kind of day I was going to have. Some mornings I could not predict whether bending to tie my shoes would go fine or send me sideways. What I eventually learned — first as a patient, and later in the research — is that chronic back pain is rarely just a local, mechanical problem. It involves the nervous system as a whole. Persistent pain changes how the central nervous system processes signals, and that dysregulation is a major reason pain can persist long after tissue has healed, alongside higher rates of anxiety, depression, and poor sleep in people living with it.
This is exactly why yoga helped where core strengthening alone had not. Yoga is one of the few interventions that addresses both the physical and the neurophysiological sides of pain at once. A randomized controlled trial of 313 adults with chronic or recurrent low back pain found that a 12-week yoga program led to meaningfully greater improvements in back function than usual care, with benefits still measurable a year later. A meta-analysis pooling multiple randomized trials found consistent short-term improvements in functional disability, and the American College of Physicians now lists yoga as a recommended first-line, non-drug option for chronic low back pain — to be tried before reaching for medication.
What that looked like for me, practically, was not dramatic backbends. It was slow, controlled movement that taught my nervous system my back was safe to move again. Cat-cow, supported bridge, and a lot of very unglamorous breathing through discomfort instead of bracing against it. The pain did not disappear overnight. But function came back before pain fully resolved — which tracks with what the research shows: yoga’s clearest wins are in restoring what your body can do, with pain relief often following behind.
Rebuilding an Ankle After a Fracture
A fracture does not just take away bone density and range of motion. It takes away trust. After mine, I remember being genuinely unsure whether my ankle would hold me on uneven ground, long after imaging said the bone itself had healed.
That gap between healed and trustworthy is proprioception — your nervous system’s sense of where your joint is in space without having to look at it. Proprioceptive deficits are well documented to persist after an ankle injury even once pain and swelling have resolved, which is part of why reinjury rates after ankle sprains and fractures are so high. Rehabilitation protocols built around balance and proprioceptive training — single-leg holds, unstable surfaces, slow controlled range of motion — are specifically designed to retrain that sense before returning to full activity.
Yoga turned out to be an unusually good delivery system for exactly that kind of training. Poses like tree, half-moon, and warrior III are, at their core, single-leg proprioceptive drills, held long enough for your nervous system to relearn what stable feels like. I did not start there. I started holding a wall, wobbling, and using words with my ankle I won’t repeat here. But rebuilding that trust, joint position sense first and strength second, is what let me eventually get back to hiking without thinking about every root and rock underfoot.
What Yoga Does for an ADHD Brain
This is the part I get asked about the most, because it surprises people: yoga is one of the only things that reliably quiets my ADHD brain. Not by forcing focus — I cannot white-knuckle my way to stillness — but by giving my mind a body to come back to.
Interoception is the sense of what is happening inside your own body — your heartbeat, your breath, tension you’re holding without noticing. There is emerging research suggesting interoceptive processing works differently in ADHD, which may be part of why sitting still with your thoughts and nothing else can feel so much harder than sitting still with a physical task in front of you. In a randomized trial of adults with moderate depressive symptoms, yoga that specifically emphasized interoceptive awareness, rather than movement alone, produced measurably better sustained attention than movement-focused yoga alone — and both forms improved mood. Separate research on adults with ADHD has found that yoga-based movement, unlike pure aerobic exercise, offers a specific benefit for temporal impulsivity that cardio alone does not replicate.
Here is my honest, unscientific translation of that research: yoga does not ask my brain to stop thinking. It gives my brain a job — track this breath, find this edge of balance, notice this stretch — that is specific and embodied enough to out-compete the noise. My mind does not go quiet because I am forcing it to. It goes quiet because I finally gave it something more interesting to pay attention to than its own static. That is interoception doing exactly what the research describes: pulling attention back into the body instead of leaving it to loop in the head.
The Nervous System Thread Underneath All of It
Back pain, ankle instability, and an ADHD brain that will not power down are three different problems on paper. But they share a common thread: a nervous system that is either overwhelmed, undertrained, or both. Yoga’s breathing practices, particularly slow inhales with extended exhales, stimulate the vagus nerve, the primary pathway of your parasympathetic, rest-and-digest nervous system. Reliable evidence links this kind of vagal stimulation to lower cortisol, improved heart rate variability, and reduced self-reported anxiety. That is not a separate mechanism from the pain relief or the proprioceptive retraining — it is the same nervous system, approached from a different angle.
What I Tell Patients
I am not going to tell you yoga fixes everything, because it does not, and the research is honest about its limits — much of it is still small or short-term, and evidence for conditions like fibromyalgia and neck pain is still developing. What I will say, as both a provider and as someone who has used this for my own back, my own ankle, and my own overactive mind, is that it is one of the few interventions I know of that works on the physical, the neurological, and the emotional layer of a problem simultaneously, without a prescription pad involved. That is worth taking seriously, whether you’re recovering from an injury, managing a mood disorder, or just trying to get out of your own head for twenty minutes.
